Mandatory Modules and Forms

Pre-Participation Physical Exam Form

All new Ontario Tech varsity athletes are required to have a pre-participation physical exam completed by a Physician.

Please retain the physical copy of the form to submit to the Athletic Therapy staff.  A copy can be submitted in person at the Athletic Therapy Clinic, located in the Campus Health Centre or via email to Athletictherapy@ontariotechu.ca.

Appointments are available with doctors at the Campus Health Centre.  To book an appointment call - 905-721-3037.  Note: There is a fee associated with this visit.

The form must be fully completed, stamped, and signed by the Physician.

Athlete Concussion Education Course

The concussion education module must be completed by all athletes prior to their first varsity season.  Please select Ontario Tech University from the list at the end of the seminar so our staff receive a record of your certificate.  Ensure you download a copy and retain it for your records.

All athletes need to ensure their certificate is uploaded on their athlete profile on the Ridgebacks Hub. 

Returning athletes will be provided with other concussion education resources and do not need to complete the seminar again.

Safe Sport

All varsity athletes are required to complete the Safe Sport Training module through the Coaching Association of Canada Locker.
