Are you having an event that you would like to request prize donations for? If so, please fill out this online form, email or fax your donation request to the UOIT Athletic Department a minimum of two weeks before your event.
If sending an email or fax, please include the following on the request:
Date of request
Date of event
Information and purpose of event
What type of donation you are looking for and how it will be used
Contact information
Please note - all donations must be picked up from the UOIT Campus Recreation Centre and will not be mailed or shipped out. Donations will be in the form of tickets or goods only, cash donations are not available.
